When intimate life changes—less desire, less satisfaction, or the feeling that “something is no longer the same”—it does not only affect sexuality; it also impacts self-esteem and the relationship. Nguyen Thanh and collaborators published a phase I/IIa clinical trial in Stem Cell Reviews and Reports (2021), with a 12-month follow-up in 31 people with decreased sexual activity, evaluating intravenous infusion of autologous mesenchymal cells derived from adipose tissue at a dose of 1 million cells per kilogram of body weight. No severe adverse events were reported in the study and, after treatment, all participants reported a better perception of their sexual satisfaction. In men, testosterone levels also increased shortly after the procedure and remained elevated for up to 6 months, before declining again by month 12. In women, no significant changes were observed in AMH, FSH, or estradiol.

Erectile Dysfunction

In this phase I clinical trial (17 men with erectile dysfunction after radical prostatectomy), a single intracavernosal injection of autologous regenerative cells derived from adipose tissue was reported as well tolerated, with minor events at one month (related to liposuction and injection) and no serious events during the 6-month follow-up. During the study period, 8 of 17 participants recovered erectile function with the ability to complete intercourse.
When erectile dysfunction appears after radical prostatectomy, it does not only change intimacy: it can also affect confidence, the relationship, and quality of life. Haahr and collaborators published an open-label clinical trial in eBioMedicine (2016) in 17 men with erectile dysfunction after radical prostatectomy who had not responded to conventional treatments.
The study evaluated a single intracavernosal injection of autologous regenerative cells derived from adipose tissue, freshly obtained after liposuction. The procedure was well tolerated: only minor discomfort related to the liposuction or injection was reported during the first month, and no new events appeared in later follow-up visits.
At 6 months, 8 of 17 patients recovered erectile function with the ability to have sexual intercourse. In the group of continent men, 8 of 11 showed significant improvement in the IIEF-5 score, with a median increase from 7 to 17; in contrast, no recovery was observed in patients with incontinence. In simple terms, this study showed an encouraging signal, especially in continent men, because several regained useful sexual function after treatment.

In addition, the sexual function questionnaire showed improvements in orgasm, satisfaction, and pain scores during the first months of follow-up.
